This exciting local pilot is part of a nationwide project, aimed at putting Community Health and Wellbeing Workers at the heart of primary care.
Our Community Health and Wellbeing Workers (CHWW) will visit households, identified by Benhill and Belmont GP Centre, at least once a month. They build a trusted relationships with these households and are a reliable and permanent point of contact for holistic support to the entire household offering community medicine at scale.
In this nationwide project, known as the Brazil Model, the role of the Community Health and Wellbeing Worker is to listen and offer help to residents/patients including:
- discussing and listening to what is important to residents and their families
- helping them find and connect with the right services
- letting them know what prevention opportunities they might be eligible for, for example cancer screening or vaccinations
- explaining how services, including housing or benefits work
- assisting them to make or attend appointments
- telling them about and linking them to resources in the community that they may not know about.
- checking in on them following a hospital discharge
- connecting people to each other in the community and so much more!

CHWW Update 4th July 2025
Community Health and Wellbeing Workers (CHWW) pilot project is coming to an end and will close by 31st July 2025.
This programme was a pilot scheme with funding for 12 months - which means it ran in a small area, to help the NHS understand how the service might be rolled out borough wide with more funding. We are hopefully that the learning from this pilot will help shape more new and innovative services to the new NHS 10-year plan.

We achieved a huge amount within our pilot working and meeting 183 Households with 508 individuals who were invited to join the program.
Participants were primarily both patients of Benhill and Belmont GP Centre and living on the estate with the option provided to opt out of working with us at any time. It was great news to see that only 21 households opted out of working with us.
CHWW’s visited the household monthly and built amazing, trusted relationships which many found made a difference to their lives. They talked about health matters, we met the police to talk about drugs and safety in the neighbourhood, housing to help with issues over their homes and other charities regarding finance, debt and support. Over time, we had 88 people that we consistently worked with, providing an individual link between people and their services, communities. Regional evaluation and our initial review shows this community health response helped people with improved mental health, physical activities and increased knowledge of services, support and direct connections to key health workers. The wider support also aimed to improved environmental factors within the physical environment and safety. A full review of the CHWW work is being undertaken on the impact for individuals, in partnership with Roehampton university. Once completed this report will be shared.
