Health Navigators

Addressing health inequality in Sutton.

NHS South West London Integrated Care System logoOur Health Navigators project is designed to address health inequality in Sutton. Age, ethnicity, place of residence, and lifestyle choices all contribute to variations in health outcomes among the local population. Through health education and early intervention, we aim to make a significant difference to individual health outcomes.

This project is funded by NHS South West London Integrated Care System.

Health Navigators Roundshaw Event July 2023What We Do

Our project recruits and trains a dedicated group of volunteers who engage with those most in need. These volunteers provide support and guidance to residents through health and wellbeing events, health checks (e.g., blood pressure), health education awareness (e.g., cancer, diabetes, blood pressure, mental health, etc.), signposting to other services (e.g., British Heart Foundation, Stroke Charities, Weight loss, etc.), and NHS app training (helping access the NHS app).

We run a busy outreach programme including:

  • GP surgeries, health centres and medical practices
  • Leisure centres
  • Health and wellbeing community events
  • Community groups

How We Help

Our volunteers provide a positive and empowering experience for those most in need. Through our Community Health Project, we aim to make a real difference in the lives of Sutton residents. We believe that early intervention and support can have a huge impact on health outcomes and quality of life.
