Health Navigators: blood pressure checks are helping to prevent disease and save lives

How health and wellness checks can pick up concerning high levels of blood pressure, and can identify potentially life-threatening conditions.

Since its launch in May 2023, the Health Navigators project has carried out 2,855 Health and Wellness Checks as of end of 2024, significantly improving community wellbeing. By offering checks in accessible locations, the project focuses on early detection of health concerns, enabling timely interventions and appropriate care. 

On 2nd December 2024, a Health Navigators session took place at Hackbridge Medical Centre from 10 am to 1 pm. The Health and Wellness Checks were conducted in the GP waiting room. Residents were invited through a text message sent three days prior, encouraging them to attend. 

During the session, 31 Health and Wellness Checks were completed. Three residents were identified with concerning abnormal results. 

Case Highlight: Early Intervention for a Resident 

  • Resident Profile: A gentleman in his 70s with liver cirrhosis.
  • Assessment Findings: Blood pressure reading of 170/110, indicating concerning high levels.
  • Immediate Action: Lisha Varghese, the project coordinator, flagged the result to the staff at the practice.
  • The resident was seen by a GP within one hour of this action and he was put on 24-hour blood pressure monitoring to properly assess the issue so that correct medication can be prescribed. 

This timely response demonstrates how Health and Wellness Checks can identify potentially life-threatening conditions. Without this check, the high blood pressure might have gone undetected, risking severe complications. 

The Health Navigators project continues to provide valuable Health and Wellness Checks, leading to early interventions and improved outcomes. The session at Hackbridge Medical Centre highlights its role in safeguarding community health and ensuring residents receive timely care.
